Patient Forms

We all know that forms and paperwork are a tedious, but necessary, part of today’s healthcare environment. Our team at Hodge Dermatology have worked hard to make the process as easy as possible for all of our patients. 

You will now receive an appointment confirmation from us through text and/or email. Clicking the link to confirm your appointment will automatically lead you to complete your patient registration and prompt you through the entire process. It’s easy to fill out, and we will remember your answers for your next visit so you only have to make necessary changes in the future. 

We are a paperless office so there is no need to fill out paper forms by hand or print and bring into your appointment. All patient intake is done electronically (saving time and trees)! You can even pay your co-pay and/or any outstanding balance from your own personal device! 

We ask that all patients register before coming into the office through their mobile device or computer. If you do not have access to a mobile device or computer, please arrive a little earlier to fill out your pre-visit information on our electronic iPads.

Getting Here
We’re located in Fullerton on Harbor Blvd. just down the hill from St. Jude Hospital (where Dr. Hodge is on staff). The two Fullerton Towers are in the “V” formed where Brea Blvd. and Harbor Blvd. join together. We’re on the third floor of the north tower.


Parking Information
There is ample free parking in the lot surrounding the building.
Additional Information
When you park, enter the lobby, right off of the patio outside. Take the elevators located to the right of the lobby. We are located on third floor.

How To Prepare

Providing you with a comfortable, efficient and complete dermatology exam is very important to you and to us. You can help us help you address your skin care concerns and achieve your skin care goals by doing a few simple things before your visit:
  • Conduct a self exam to identify any moles that concern you or have recently changed.
  • Write down several questions to ask your dermatologist or allergist.
  • Complete your paperwork in advance online. If you cannot complete it online, please arrive 15 - 30 minutes early to complete paperwork.

Questions To Consider & Ask During The Consultation

Before you come and see us, make notes of anything that you have concern over.
By doing a self exam of your skin, you can then make notes of anything that concerns you. Any moles, or spots that have changed in the last year are a good idea to bring up with your Doctor.
No question is too small, we are here to help. We want to make sure that you leave our office with complete confidence that we made enough time to go over all your concerns.
Feel free to ask about what skin care regime or even what sunscreens would work best for your skin type; this is important for where we all live, sunny Southern California.
